Please read the below Consent Waiver and complete the form. Submission of this form is required prior to all virtual or live Candle Workshops or other classes.
 

RELEASE/WAIVER: I hereby agree to indemnify and hold harmless PUT ON LOVE + MARKET and its staff against all claims for personal injuries or damages of any kind arising from participation in all virtual or live Candle Workshops or other classes and their resulting products.

NOT MEDICAL ADVICE: I understand that no ingredients, ingredient descriptions, or guidelines presented are intended to treat, prevent, or cure any disease or condition. We are not a doctor licensed in any state and do not intend to provide any medical treatment or advice on mental and physical health.

PHOTO CONSENT: I agree that PUT ON LOVE + MARKET may use my photograph or video in the routine promotion of workshop or class activities.

SUBSEQUENT VISITS: This waiver shall be kept on file and adhered to for both initial and subsequent visits to the live or virtual workshop or class by the participant.

NO REFUND POLICY: I fully understand that all virtual or live workshops or classes provided by PUT ON LOVE + MARKET offer a No Refund, No Cancelling Policy for absence of my participation. When I miss my scheduled workshop, I understand that I will need to re-purchase a new session.

I hereby voluntarily agree to release, waive, discharge, hold harmless, defend, and indemnify PUT ON LOVE + MARKET, its staff and assistants from all claims, actions or losses for bodily injury, property damage, and any incidents or accidents, which may arise from my participation in any workshops or classes, the resulting products, or from use of equipment provided by PUT ON LOVE + MARKET. I fully understand and acknowledge that the workshops or classes offered at PUT ON LOVE + MARKET may involve inherent risks and hazards which may result in injury or illness due to the potentially dangerous nature of high temperature waxes, wax melting equipments, and jars.

I am over 18 years of age, and accept the full liability for creations made during and after all virtual or live Candle Workshops or classes. By submitting the form below, I agree to the terms stated above:
